Other Comments: This article is very timely given the mounting perception internationally of the need to curtail rising health care costs without compromising necessary care to individual patients. The searches and their results reported in the article will be found very useful to health managers and policy makers.
The article would benefit, if possible, from several clarifications and corrections:
1. The bibliography is anomalously truncated at citation # 23. However, there are 35 numbered citations in the text.
2. Although, starting with the title, the manuscript refers to a "3-step strategy", the 3 steps are nowhere explicitly identified. Ideally they should be enumerated after the 2nd sentence of the abstract and again in the Results section of the body of the article, probably under the header: "The proposed model". Are the three steps "MBB", "PBB" and "application of criteria"? This is nowhere made explicit. A table or box insert enumerating the 3 steps could be a useful way of getting the 'bottom line' across to the reader.
3. The exposition would be even clearer if the relationship between the model being proposed and the searches, surveys and the results of reviewing the information gleaned from them were elaborated more explicitly. For example, was the MBB/PBB concept developed before the literature review and surveys were performed, or was it derived in the course of them? This is not currently clear.
4. Related to the last point, it could have been elaborated more clearly the extent to which current protocols in various countries, like Germany and Norway, conform to the proposed model, and in what ways they differ.
