    This is a timely paper. With the shift away from tracheal/ intubation to nasal-care devices in preterm care the problem of increased sepsis in nasal-cared preterms is coming up. This paper points a way to that mechanism and its prevention.Indeed I saw a similar case in our level lll NICU in a nasal-cared preterm.

     

    The pressure effect of nasal care devices and more handling of nasal area, leads to increased colonisation of nasal skin with staph. Due to pressure effect on skin organisms invade easily the lymphatics and blood in that area. Increased attention to asepsis in nasal-cared preterms in nasal area similar to tracheal care and use of oro-gastric feeding when possible are required.
