I really appreciate the Author’s attempt to show something new in the field of disaster medicine. In fact, we usually find only articles on “state of art” , cases report, protocols, triage, caveats and never studies! As to my opinion, only new ideas really make the difference in medicine. Probably many reviewers are biased against this new modality for acute pain management. I definitely don’t agree with those who think that bad pain management during disaster events is not a serious problem. On the other hand, there are no hyperbaric chambers during disaster events and probably only a military sailor could have these maybe on aircraft-carriers….. But I am really enthusiastic about this original study. At last something new! Furthermore, I agree with the author for the necessity to trigger the research for further investigation on hyperbaric properties.

Although a partial lack of scientific methodology, this study seems to be original and interesting for the utilization of the hyperbaric chamber to try to manage acute pain during disaster events.
The need to share scientific knowledge related to disasters is very important and we need also appropriate research on acute pain management. I agree with Dr. M. Bortolin ( Bortolin M, Ciottone G, Codreanu TA, Delooz HH, Photiou S, Barletta C, Keim M. Disaster Medicine: Perspective . WebmedCentral DISASTER MEDICINE 2012;3(4):WMC003219) for the fact that in comparison to those challenges faced by other specialties, disaster medicine researchers often face significant challenges involving lack of accurate documentation during events, logistical difficulties in data collection, a lack of standardization for comparability and analysis of like events. Thus, the importance of this article can be seen in the attempt to try to show something new for multimodal pain management during calamities.

Hyperbaric management of pain is not a new finding, the existent literature being sufficiently abundent. Where this article originality lies is that hyperbaric therapy was used during the recovery phase of a disaster, with reported good results. However, the methodology lacks rigour and the random selection of patients is not robust enough. Consequently, the results may be excessively biased, although, again, hyperbaric management for painful conditions is not a new concept.
From a disaster management perspective, the logistics of deploying and running a hyperbaric chamber for this sole purpose would, I suspect, be regarded as a excessive.
The command of English language needs reviewing. Also, I am not convinced that the author understands what "conflict of interests" means.

I believe the paper whould have been better categorized in The "Pain" section. The fact that the patients were seen during military mission abroad is not relevant to the topic.
From this retrospective study no conclusion can be drawn. Methods are not sufficiently standardized to provide reliable data.
Reference list is inadequate since it does not report even a single study on the analgesic effect of hyperbaric oxygen, and there are many...

Strenghts
This study it is interesting about the use of the hyperbaric oxygen therapy in the treatment of the wound in the field of the disaster medicine.Weaknesses
- the method of selection of the patient: which is the method to include a patient in the “hyp” treatment?
- the groups are similar only for the anthropomorphic data and not other variables that could influence the type of the wound and the healing (potential confounders), therefore the results could be innacurate
- the number of patients presented in the study is low
- the presentation of the results and the tables are not completely clear. -

Limits:
A possible relationship between hyperbaric oxygen exposition and reduction of pain should be better investigated through a perspective randomized clinical trial. On the contrary, this is a simple "retrospective" analysis.
