    This is an isolated case report of a 68 year-old patient with visual synesthesia after an auditory stimulus. His past medical history was significant of essential tremor, stroke, hemorrhagic pituitary (with hypopituitarism secondary) and several vascular risk factors with clinical data of arteriopaty (iliac and renal angioplasties, and bilateral carotid stenosis). This case only describes a cranial MRI showing in addition to empty sella, areas of demyelination in the basal ganglia area and base of the ponts.
    The authors make no reference in the description of the clinical case of laboratory studies demonstrating the electrolyte normality despite being treated with mineralocorticoids. Neither neuroimaging concerns controls assess the development of demyelinating lesions, but the author believes that these lesions are due to a pontine and extrapontine myelinolysis.
    This article argues that properly synesthesia are not due to alterations in the optic nerve (as happens in the cases cited in the bibliography), but in my opinion not clearly demonstrated the relationship between synesthesia and demyelinating lesions. I think it can not be excluded, based on paper information that the demyelination may be due to a small vessel atherosclerotic disease (leukoaraiosis).
    As the above reasons, I think that this paper should not be accepted for publication unless justified more clearly the nature of the ischemic lesions.

    The authors reported an uncommon case of sound induced photism in a 68 years old male. They provided a brief background of the disease which allow readers to follow the symptom and signs easily. The conclusion linked the gamma band activity at the sensory cortex to be a common source for the symptoms we seen at this patient. Although there is no direct proof for this, it stand as a possible explanation for the case. However, if the authors could provide the quantity( noise/tone?, intensity ?dB, duration, frequency and characteristics) of the sound by which the patient is being induced to develop photism, this report will be more complete.

    we exposed rats to different sound using AEP to record and watch for the responses. We also recorded gamma band activity at the auditory cortex.
