  • Do the results support the claims? If not, what other evidence is required?

    Yes

  • If a protocol is provided, for example for a randomized controlled trial, are there any important deviations from it? If so, have the authors explained adequately why the deviations occurred?

    No

  • Is the methodology valid? Does the paper offer enough details of its methodology that its experiments or its analyses could be reproduced?

    Yes

  • Would any other experiments or additional information improve the paper? How much better would the paper be if this extra work was done, and how difficult would such work be to do, or to provide?

    No

  • Is this paper outstanding in its discipline? (For example, would you like to see this work presented in a seminar at your hospital or university? Do you feel these results need to be incorporated in your next general lecture on the subject?) If yes, what makes it outstanding? If not, why not?

This paper would be an outstanding continuing eduacation course for dentists involved in implants.

  • Other Comments:

The authors describe their belief that modern dental surgery achieves better results through the combination of regional anesthesia and conscious sedation techniques.

The authors also properly state the importance of the proper selection of patients and various other important factors in reducing the incidence of complications.  The patient must be completely informed about the technique and must actively collaborate for the surgery to be successful.  This article is useful for anyone performing implant surgery.

  • What are the main claims of the paper and how important are they?

The use of conscious sedation and local anaesthesia is an alternative to general anaesthesia for the placement of zygomatic implants. This is an important concept as it will reduce the reliance on general anaesthesia in these patients

  • Are these claims novel? If not, please specify papers that weaken the claims to the originality of this one.

Yes. I have not seen this described before

  • Are the claims properly placed in the context of the previous literature?

    Yes

  • Do the results support the claims? If not, what other evidence is required?

This is a report of 2 cases that have been successfully manged. provided that it is viewed as a case report it is apporpriate

  • If a protocol is provided, for example for a randomized controlled trial, are there any important deviations from it? If so, have the authors explained adequately why the deviations occurred?

    NA

  • Is the methodology valid? Does the paper offer enough details of its methodology that its experiments or its analyses could be reproduced?

    NA

  • Would any other experiments or additional information improve the paper? How much better would the paper be if this extra work was done, and how difficult would such work be to do, or to provide?

Turning this into a case series would improve the validity,  but as a case reoprt it is fine

  • Is this paper outstanding in its discipline? (For example, would you like to see this work presented in a seminar at your hospital or university? Do you feel these results need to be incorporated in your next general lecture on the subject?) If yes, what makes it outstanding? If not, why not?

I will be using this in a lecture

  • What are the main claims of the paper and how important are they?

The place for sedation in the dental office for oral surgery is evidence-based... but still complicated to admit in some parts of Europe. This article is a case-review and points out one indication of a well controlled conscious sedation associated with effective local anesthesia.

  • Are these claims novel? If not, please specify papers that weaken the claims to the originality of this one.

No novelty in the US or the UK. However, a stilll to be discovered field in latin Europe. Conscious sedation for oral ambulatory surgery supported by a reknown team of anesthesists in Padova, one of the most prestigious  medical school in Europe, I go for it...

  • Are the claims properly placed in the context of the previous literature?

    Yes, english speaking litterature.

  • Do the results support the claims? If not, what other evidence is required?

    Of course.

  • If a protocol is provided, for example for a randomized controlled trial, are there any important deviations from it? If so, have the authors explained adequately why the deviations occurred?

    N/A

  • Is the methodology valid? Does the paper offer enough details of its methodology that its experiments or its analyses could be reproduced?

    Yes

  • Would any other experiments or additional information improve the paper? How much better would the paper be if this extra work was done, and how difficult would such work be to do, or to provide?

Any other case report on same subject.

  • Is this paper outstanding in its discipline? (For example, would you like to see this work presented in a seminar at your hospital or university? Do you feel these results need to be incorporated in your next general lecture on the subject?) If yes, what makes it outstanding? If not, why not?

This is a case-report. It therefore needs to be added to the existing literature. However, coming from Italy, it means that experienced italian anesthesists support CS for ambulatory dentistry. This needs to be pointed out for southern Europe.

  • Other Comments:

Europe (apart from the UK) needs similar abundant case reports to support the CS + RA combination for ambulatory oral surgery.

  • What are the main claims of the paper and how important are they?

Having a patient that is sedated and comfortable is the main application of this article. The use of conscious sedation is becoming more prominent for the general practitioner or specialist to utilize in their practices.  The importance of profound local ansesthsia is a key component in a successful outcome for both the patient and practioner - as is mentioned in this article.  The claims in this article are relevant and useful topic.

  • Are these claims novel? If not, please specify papers that weaken the claims to the originality of this one.

Although not novel, the claims in this article are important and serve the reader well.

  • Are the claims properly placed in the context of the previous literature?

    Yes

  • Do the results support the claims? If not, what other evidence is required?

    Yes

  • If a protocol is provided, for example for a randomized controlled trial, are there any important deviations from it? If so, have the authors explained adequately why the deviations occurred?

    NA

  • Is the methodology valid? Does the paper offer enough details of its methodology that its experiments or its analyses could be reproduced?

    Yes

  • Would any other experiments or additional information improve the paper? How much better would the paper be if this extra work was done, and how difficult would such work be to do, or to provide?

The article is a case report and is well written.  The details are well documented as to the medications used during the sedation as well as the pre-operative medications used.

  • Is this paper outstanding in its discipline? (For example, would you like to see this work presented in a seminar at your hospital or university? Do you feel these results need to be incorporated in your next general lecture on the subject?) If yes, what makes it outstanding? If not, why not?

The concept of using sedation in the dental office is a subject that is offered at seminars and courses, etc.

  • Other Comments:

The article was well written and informative.  The utilization of conscious sedation in the dental office is an important component that is available to general dentists and specialists that have not had formal training in the field.  After completing either an oral sedation course (or as in this article) an intravenous sedation license course, the placement of implants and various other procedures can be achieved with the patient comfortably sedated.  This article may enlighten those practioners who have no training in sedation to seek out the required courses and put this new found education to use.
