Young women and Cerebral Venous Thrombosis
Posted by Prof. Jera Kruja on 14 Aug 2013 12:26:39 PM GMT

  • What are the main claims of the paper and how important are they?

The main claim of the paper is the review of literature on Cerebral Venous Thrombosis in young women.

  • Are these claims novel? If not, please specify papers that weaken the claims to the originality of this one.

This is not a new claim.

  • Are the claims properly placed in the context of the previous literature?

    Yes, there are.

  • Do the results support the claims? If not, what other evidence is required?

    Yes

  • If a protocol is provided, for example for a randomized controlled trial, are there any important deviations from it? If so, have the authors explained adequately why the deviations occurred?

    NA

  • Is the methodology valid? Does the paper offer enough details of its methodology that its experiments or its analyses could be reproduced?

    NA

  • Would any other experiments or additional information improve the paper? How much better would the paper be if this extra work was done, and how difficult would such work be to do, or to provide?

More attention to the description of the personal cases.

  • Is this paper outstanding in its discipline? (For example, would you like to see this work presented in a seminar at your hospital or university? Do you feel these results need to be incorporated in your next general lecture on the subject?) If yes, what makes it outstanding? If not, why not?

    NA

  • Other Comments:

    The authors described the:

    1. Epidemiological data using recent and important references.
    2. Risk factors are described in details, systematically introduced, too.
    3. The clinical signs of cerebral venous thrombosis are presented in a classic hierarchical ranking, thus highlighting the most important clinical features.
    4. The diagnosis criteria are updated.

There are a small number of patients included in the study (2011 – 2012), but their young mean age (corresponding to the reproductive period) and the higher number of patients on the postpartum period, make the study interesting.

I found useful the references used and their mention in the text.

  • What are the main claims of the paper and how important are they?

The authors have tried to highlight importance of cerebral venous thrombosis incidence in young female by giving their data but unfortunately they have written the whole article like a review article without presenting any of their findings and only in the abstract they give their findings. They should have written it with proper materials and methods giving how many patients they found and what investigations they performed-course of management and what treatment they gave like lmwh/oral anticoagulants and what risk factors on investigation they encountered-rather than just giving 8 post partun case ,2 pregnant case ,1 patient on oc and one with positive antiphospholipid syndrome-what investigations they did CT/MRI etc any LP or not D-dimer or not i can understand thjeir language problem but just to copy from another article without giving your findings doesnt make any aense-this os literally the same as CVT from Piazza G who has tried to explain his case report on that basis but they have not given their findings in detail at all even if due to language problem they use same language but they sdhould describe what they treated bein g neurologists.

yes the author should rewrite giving his detailed findings of the patients treated as materials and methods and highlight the importance of diagnosing CVT in YOUNG FEMALES as although important in young female population on ocs or in reproductive age group he should highlight what makes prognosis different in males and what is the aim of writing this article as the article is important but not the way it is written just copying causes of CVT like in a text book

with the advent of ART and lot of use of oral contraception and sudden appearence of headache in a younggirl with CVT being 3/4th common in females as per american heart association recommendations it is imperative that simple headache during pregnancy be not ignored or like the case presented by piazza which was a CVT due ti OC'S precipitating an underlying factor V lDEN DEFICIENCY should promote awareness amongst people dealing with IVF/pregnancy as recommended by AHA that a teamworl of physician and early diagnosis can help 90%success or more in females who are the 3/4th population affected.
