Post Operative Wound Infections Following Implant Surgeries
Posted by Dr. William J Maloney on 25 Apr 2014 11:23:09 PM GMT Reviewed by Interested Peers

  • What are the main claims of the paper and how important are they?

The main purpose of this article is to assess the relationship between theatre population and postoperative wound infection.

  • Are these claims novel? If not, please specify papers that weaken the claims to the originality of this one.

Post operative wound infection following implant surgery is a serious problem in surgical practice.  A one year prospective study was performed.

  • Are the claims properly placed in the context of the previous literature?

    Yes

  • Do the results support the claims? If not, what other evidence is required?

    Yes

  • If a protocol is provided, for example for a randomized controlled trial, are there any important deviations from it? If so, have the authors explained adequately why the deviations occurred?

    No

  • Is the methodology valid? Does the paper offer enough details of its methodology that its experiments or its analyses could be reproduced?

    Yes

  • Would any other experiments or additional information improve the paper? How much better would the paper be if this extra work was done, and how difficult would such work be to do, or to provide?

    No

  • Is this paper outstanding in its discipline? (For example, would you like to see this work presented in a seminar at your hospital or university? Do you feel these results need to be incorporated in your next general lecture on the subject?) If yes, what makes it outstanding? If not, why not?

Yes- It would be a great seminar for a group of surgeons.

  • Other Comments:

Post operative wound infection following implant surgery is a serious problem in surgical practice. A one year prospective study was performed. A thorough methods section is presented.  The results show that increased theatre population does not significantly lead to more postoperative infections. Patient age and sex have no influence on the occurrence of postoperative infection.

  • What are the main claims of the paper and how important are they?

No relation does exist between the number of the people in the OR and post operative infection, if proved, very important

  • Are these claims novel? If not, please specify papers that weaken the claims to the originality of this one.

    Yes

  • Are the claims properly placed in the context of the previous literature?

    Yes

  • Do the results support the claims? If not, what other evidence is required?

Yes but insufficient, multi-center studies with larger numbers are required as this complication consists only a part of the post-operative complications

    Yes

    1. Introduction lacks references. I suppose the facts presented in the introduction section should be supported by recent references.

    2. As they have sent all the samples to the lab, I suppose one major section which is lacked in this study is the prevalence of the different organisms involved in postoperative infection and their sensitivity to the antibiotics. This would immnesly add to the quality of the present paper. I would suggest the authors to add this data, if possible.

    3. One probable cofunding factor to this study is the other routes of infection i.e. hematogenous and so on, how were these factors eliminated from the study?

    4. The references are old and it is better to be replaced with new ones.

Yes, as sometimes teams with large number of the people are required especially in major surgeries, this fact would be a relief to all the practitioners in the field

  • What are the main claims of the paper and how important are they?

To assess the relationship if any between theatre population and wound infection after internal fixation of fractures. this is an important aspect of quality of surgical care in general

  • Are these claims novel? If not, please specify papers that weaken the claims to the originality of this one.

There have been similar findings by Onche et al as seen in their references

  • Are the claims properly placed in the context of the previous literature?

    Yes

  • Do the results support the claims? If not, what other evidence is required?

There was no statistical significance from their findings

  • If a protocol is provided, for example for a randomized controlled trial, are there any important deviations from it? If so, have the authors explained adequately why the deviations occurred?

    Yes

  • Is the methodology valid? Does the paper offer enough details of its methodology that its experiments or its analyses could be reproduced?

    Yes the methodology is valid

  • Would any other experiments or additional information improve the paper? How much better would the paper be if this extra work was done, and how difficult would such work be to do, or to provide?

The authors should rather tell us the factors that contribute to wound infection after implant surgery

  • Is this paper outstanding in its discipline? (For example, would you like to see this work presented in a seminar at your hospital or university? Do you feel these results need to be incorporated in your next general lecture on the subject?) If yes, what makes it outstanding? If not, why not?

The findings are not really novel
