    The authors did a good work in trying to sum up some of the most disturbing problems in parkinson's disease (PD). Nonetheless, I have some major remarks,

     

    1. Authors speak about the non-dopaminergic origin of many of these sympotms without giving evidence about it. This is important, specially in view of possible treatments.

    2. Some very important adverse reactions to dopaminergic therapy, i.e. impulse-control disorders, represents also clinical complexities in PD but were not included in the review.

    3. Similarly important symptoms such apathy, fall, fear to fall, etc were negleted or insuficiently described.

    4. I would have loved to see a table summing up the reviewed complexities.

    5. Some references are missing, for example in the "Voice Changing / sialorrhea" section.
