    This aarticle involves a single case report and relatively brief review of general information about Salmonella. The main emphasis seems to be the recurrence of infection (in the urinary tract) after five years, however there is no data presented that this represents persistence of infection rather than re-infection. In fact, the initial isolate was sensitive to ciprofloxacin and the subsequent isolate resistant.

     

    Many details in the case presentation do not seem to be relevant to the Salmonella infection and there is minimal discussion of management.

Salmonella typhi infection and urinary tract

    Bacteriuria due to Salmonell infection usually occurs after a recent typhoid fever or in chronic carrier. Presence of Salmonella typhi in the urinary tract is commonly associated with localized anatomical abnormalities, systemic diseases and immunosuppressive state. Eradication in such cases are usually difficult. The clinical course as mentioned in the case report was expected and well known from literatures. 

     

    More specific comments as below:

    1. Introduction: The introduction was general knowledge on salmonella typhi. Instead the author should extensively review the available literatures on salmonella typhi and urinary tract and provide the rationale for reporting the case.

     

    2. Case report: The authors did not provide details on how the diagnosis of acute renal failure was made. Similarly the authors mentioned on chronic renal failure but no further details were provided. The treatment and outcome of the acute renal failure was not mentioned. What was the significance of 7.5cm x 4.1cm mass mentioned from the ultrasound examination? Why did the patient develop multiple left sided kidney stones? Were other causes of nephrolithiasis investigated in this patient? Details on the surgery for removal of stones were not mentioned. Was there any repeat culture after the second course of ampicilin-sulbactam? What happened after the surgery? The second encounter after 5 years was due to admission for acute urinary tract infection. What happened to the chronic renal failure? Why was mitral valve prolapse mentioned or more specifically did the authors attempt to exclude possibility of vegetation using the more sensitive transoesophageal echocardiography?

     

    3. Discussion: Again the authors merely provided general discussion on typhoid and salmonella typhi. Instead the authors should base their discussion on the case – what were the interesting points about the case, what were the limitations in the management of the patient and so on. Authors should also review available literatures on antibiotic resistance and how is this information applied to this patient.
