    I agree that faculty structure can dictate the residency training, but the key issue is TIME spent doing a certain activity, not just having the faculty.  The reason you saw that correlation is that those are the subspecialties that the residents spend the least time on as well.  I speak from the urogynecology perspective since I am one.  Generally the programs have a one or two month urogynecology rotation in the entire 48 months - You cannot expect the residents to gain a lot of experience there.  Due to reduction is work hours further, the most time is spent on obstetrics service, followed by outpatient clinics and general gynecology.  MFM rotations are built into obstetrics and come as the residents become more senior.  Due to strict criterion, residents spend about 6 months on gyn oncology and 4-6 months on reproductive endocrine.  They spend the least amount of time on urogyn and therefore, despite volumes, do not make use of the experience available.  Therefore, in my mind, the solution is a track system to better prepare the ones that are going to be surgeons for surgery and the ones that are going to be obstetricians for labor and delivery suites.

    Needs more discussion  about recomendation for standerdization of residency experience in order to assure equality in training inorder to lead to more uniformity in services to patients after residency training is complete.Clearly itner-residency cooperation is needed and since residency training has been reduced in this country to 80 hour a week residency should be extended to 5 years inorder to allow adequate surgical training in a specialty that has declared itself to be primry care and added a focus on clinic care at about the same time resident hours were deceased resulting in a decrease in surgical training. The only operative primary care specialty that I am aware of.

    Opinions and knoledge but no experience. I was trained in a era that we were specialist and had more extensive surgical training and much longer hours and opertunity to see more pathology.
