This is a well written manuscript however there is a lot of duplication between the abstract and the introduction. In my opinion the abstract section should be in the form of structured abstrat as follows:

Abstract:

"Back ground"

"Aims"

"Patients, Materials and Methods"

"Results"

"Conclusions"

Key Words

In my opinion if the abstract is re-written to provide a short back ground to the study and the rest of the abstract is written as a summary any one who reads the abtract without reading the entire manuscript would clearly understand what the study was about as well as the results and conclusion. Repetition of general information in the abstract and introduction would make reading of the manuscript laborious. If the abstract is rewritten I have no doubt that the manuscript would have a high ranking. I hope the authors would provide a rewritten structured abstract to replace the current abstract.
