    Title: “Severe Hypothyroidism, Coronary Artery Disease on CT Coronary Angiography and Hypoperfusion on Contrast Echocardiography”.

    Dear Editor,

    We have read through this case report and we think that some things should be better outlined:

    1. English should be improved by a native English speaker.
    2. The abstract conclusions seems to “strong” for a case report. I think they should be deleted or re-phrased.
    3. There is a deep concern about this work. It seems quite surprising not to read that the patient had undergone coronary angiography. The patient suffered from “typical angina on exertion”, as outlined by the authors. Why did he not undergo coronary angiography but rather the evaluations stopped to non-invasive technique?
    4. Furthermore, we have no idea about the drugs administered to the patient and their dosage. The patient had a severe hypothyroidism and chest pain on exertion: what did physicians do in order to improve such conditions? Please comment.
    5. Another ethical problem. The authors wrote: “Three-vessel proximal coronary artery disease was found with diffuse involvement of coronary arteries. (Fig.1)”. Why did the patient not undergo CABG? Did the authors ask a cardiac surgeon opinion? What did he say? Please complete the description.
    6. Again, the conclusion of the manuscript are too strong for a case report. Please delete or re-write them.

    It is presented a case report of a patient with Severe Hypothyroidism and  Coronary Artery Disease. The coronary arteries and miocardial motion are investigated through CT Coronary Angiography and Hypoperfusion on Contrast Echocardiography.

    No. The inkages between hypothyroidism and coronary artery disease are investigated by many researchers. It is obvious even in a ordinary cardiology manual.

    Not novel. Quite a few studies in this area have been performed. Pubmed search will reveal these.

    No details of the protocols and procedures have been described and provided so that the study can be reproduced. This case study just mentions the procedural techniques used in the descussion section.
