    Population: An experimental rat study investigating the response to surgically inflicted trauma on 56 laboratory rats.

     

    Intervention: Surgically inflicted fracture of the right femur under GA.

    4 fractures had intra-medullary K-wire fixation in a posterior position (the significance or reasons for this are not clearly explained).

    The selection or allocation process is not described nor is the physical characteristics or ages of the rats in each group. 

    14 rats excluded because of infection.

    Rats sacrificed at 48, 96 or 168 hours. The numbers in each group are not reported. Assuming equal numbers: (56 - 14)/3 = 14 in each group.

     

    Comparison: Healthy left femur of (?)the same rat.

     

    Outcome: An objective (DNA/mg) but surrogate marker of bone remodelling and cell proliferation was used. The statistical methods used are not described (only the statistical package) and raw data is not presented, so the reader is unable to check if the data supports the conclusions.

     

    Reducing bias and confounding: It is not described in the text if strategies to reduce bias such as blinding or randomisation were used. The experimental groups were small and a substantial number were lost to follow-up. It is worrying that despite aseptic technique 25% of the subjects were lost to follow-up because of infection.

     

    Evaluation: The effect size of the experiment cannot be calculated and neither can the significance of the findings be checked because of the lack of data presented. Tables of subject characteristics and data should be included. The experimental process appears sound but further detail or referencing of methods is required for the experiment to be replicated. For example how was the magnitude of the injury standardised across all subjects? Greater detail and clarity in the methodology is required. This is an emotive topic (using rats and sacrificing them in the name of science) IF it is to be justified then the data and outcomes produced must be valid. The quality of this paper is let down by incomplete reporting, also the relevance and generalisability of the results to human bone and non-experimental injuries is unknown.

    I had the pleasure to review the article entitled "Remodelling through the growth plate - an experimental study in rats". In the present study a substantial amount of work has been done. The purpose of this study was to measure the cellular activity in different parts of the growth plate cells after fracture. The authors have conducted a very well organised experimental protocol and their results along with the educative illustrations, are soundly supporting their final conclusion. The authors conclude that the growth plate alters its activity so as to remodel the angulation of the healing fracture. The information provided is quite interesting and innovative, especially for the orthopaedic surgeons dealing with traumatic conditions.

  • Other Comments: I would like to congratulate the authors on their work. The paper provides significant information on the function of the growth plate post trauma and on its role on remodeling. It also has great clinical applications on the treatmant of an immature skeleton.
