This is a very intersting report of vision loss in the setting of pre-eclampsia.
The case report is confusing in that it states nifedepine was used to control BP initially, and then later on states that "antihypertensives were administered." One can only assume that post-delivery, no BP meds were given. This should be clarified.
It would be good to note if IV contrast was used for the CT scan and to state why a MRI with gad wasn't performed instead (not available?). The discussion should be lengthened to discuss what "hypodense" lesions in the occipital lobe imply.
Was an afferent pupillary defect present in either eye?
There was some evidence of retinal vascular involvement per the case report, so 1) it would be incorrect to state that "Cortical blindness is a clinical syndrome characterized by intact pupillary reflexes and normal fundoscopic findings" because there were fundoscopic findings in this case and 2) a fluorescein angiogram would have helped in the diagnosis to determine if vascular compromise or stasis was the cause of the vision loss. Profound non-perfusion in the both eyes/optic nerves could have caused this patient's visual symptoms as well.

Complete loss of vision in patients with pre eclampsia and eclampsia is a rare occurance and therefore presents scientific interest for ophthalmologists as well as ginecologist. However , the mechanisms of this syndrom remain to be obscure. I didn't find author's efforts to investigate caual relations between retina and central parts of the brain. The paper demonstrates only fact without any mechanisms and explanations.
