Report on a single case of shoulder dystocia.
Posted by Prof. Muhammad Aslam on 06 Jun 2014 04:37:26 PM GMT Reviewed by Author Invited Reviewers

  • What are the main claims of the paper and how important are they?

This is a case report of a single case of shoulder dystocia. Tha author claims that good results in this case were due to some training program which the attending obstetrician has attended. This is an important claim supported by a large body of published literature on the subject.

  • Are these claims novel? If not, please specify papers that weaken the claims to the originality of this one.

Yes, the claim is novel. But many studies have already been published in the literature that have shown the importance of shoulder dystocia drills, using simulation, on the outcome of this acute obstetric emergency.

  • Are the claims properly placed in the context of the previous literature?

    Yes

  • Do the results support the claims? If not, what other evidence is required?

No, reporting a single case of shoulder dystocia and claiming that some training program is successful to acieve better results is not conclusive. A different study design will be needed to support this claim where data before and after the institution of training program should be compared to show the improved outcome after completion of program.

  • If a protocol is provided, for example for a randomized controlled trial, are there any important deviations from it? If so, have the authors explained adequately why the deviations occurred?

    NA

  • Is the methodology valid? Does the paper offer enough details of its methodology that its experiments or its analyses could be reproduced?

No, the author has not provided the details of training. In any case a different study design is needed to fulfil the objectives of this paper.

  • Would any other experiments or additional information improve the paper? How much better would the paper be if this extra work was done, and how difficult would such work be to do, or to provide?

The author can conduct a study to compare the fetomaternal outcome before and after the institution of training program.

  • Is this paper outstanding in its discipline? (For example, would you like to see this work presented in a seminar at your hospital or university? Do you feel these results need to be incorporated in your next general lecture on the subject?) If yes, what makes it outstanding? If not, why not?

    No

  • Other Comments:

The paper contains many grammatical mistakes which need correction.

  • What are the main claims of the paper and how important are they?

Throught this Case report authors are saying that it is important to train the resident doctors via various training programmes to manage shoulder dystocia which is an acute obstetrical emergency so that there is no morbidity and mortality to the mother and the fetus.

It is absolutely important claim that the resident doctors should be given the training in managing shoulder dystocia.

however the claim that the need for training resident doctors, is a traditional and age old known claim in residency programmes.

  • Are these claims novel? If not, please specify papers that weaken the claims to the originality of this one.

    Not novel .

All standard text books of obstetrics and gynaecology have this content.

  • Are the claims properly placed in the context of the previous literature?

    Yes

  • Do the results support the claims? If not, what other evidence is required?

    Yes they support

  • If a protocol is provided, for example for a randomized controlled trial, are there any important deviations from it? If so, have the authors explained adequately why the deviations occurred?

    Not applicable here

  • Is the methodology valid? Does the paper offer enough details of its methodology that its experiments or its analyses could be reproduced?

    Not applicable here

  • Would any other experiments or additional information improve the paper? How much better would the paper be if this extra work was done, and how difficult would such work be to do, or to provide?

Case presentation should have all details elaborated so that real critical analysis is done.

  • Is this paper outstanding in its discipline? (For example, would you like to see this work presented in a seminar at your hospital or university? Do you feel these results need to be incorporated in your next general lecture on the subject?) If yes, what makes it outstanding? If not, why not?

    No

    It is a good paper.

Not outstanding because it is a standard management protocol all over the world, but the promptness showed by the whole team to give a healthy baby to a healthy mother, in the end is worth appreciating

  • Other Comments:

Please continue the good work of training the residents and during the process of teaching one improves and learns . the process of teaching and training elevates the level of knowledge of all involved. This sort of training in obstetrical emergencies is a must
